Adoption Application
All potential adopters must complete a Pre-Adoption Application. To save time, you can download a Pre-Adoption Application, complete the form and either bring it to the shelter, email the form to svasmanager@srt.com, fax the form to 701.838.7622, or fill it out during your visit.
